Who Will Care for My Adult Child with Disabilities When I No Longer Can?

Coordinated Care of MetroWest will complete the puzzle.
Providing case management and advocacy services for adult children with disabilities.
Coordinated Care of MetroWest will be your loved one's advocate.
You want to ensure quality care for the lifetime of your loved one, no matter what happens to you.
Putting together the benefits, services, and care for your child is an overwhelming responsibility. You need to know that all these pieces will be put together when you are no longer able to do so, and that everything will always be taken care of according to your wishes.
With Coordinated Care of MetroWest (CCM), you have the opportunity to develop individualized plan that will provide lifetime advocacy if you become incapacitated or wish to delegate responsibility, or upon your death. This plan will be maintained and carried out under CCM’s supervision.
A Powerful Partnership
Coordinated Care of MetroWest coordinates the benefits, services, and care that your loved one requires. To accomplish this mission, it works with two respected agencies: Jewish Family Service of MetroWest and The Jewish Community Foundation of MetroWest NJ.
Jewish Family Service (JFS) of MetroWest handles the case management, which includes:
- Designating a case manager who will maintain ongoing communication with your loved one with special needs.
- Coordinating, monitoring and advocating services that may be needed from a variety of community and government agencies.
- Supportive Counseling to ensure that your loved one leads a safe and fulfilling life.
- Serving as a liaison to trust officers or other financial planners who may also act on behalf of your loved one.
Jewish Community Foundation (JCF) of MetroWest NJ manages the financial aspects of your plan; it holds and invests the money you have set aside for your child’s care, according to your instructions. Various methods of funding the plan are available.
Let Coordinated Care complete the puzzle for your child when you no longer can.
Coordinated Care of MetroWest will be there to manage the total program and achieve the best quality of life for your loved one.
